비만에서의 장내 미생물 무리의 역할

Abstract

Obesity is a critical target for public health interventions worldwide. There are many causes of obesity, and the importance of the gut microbiota in its pathogenesis has recently been recognized. The composition and function of the gut microbiota play a role in obesity and metabolic disease, yet the underlying mechanisms are unclear. Advances in our understanding of the link between obesity and the gut microbiota have suggested the potential of its manipulation for treating obesity. In this review, we summarize current knowledge of the interactions between the gut microbiota and obesity as well as the therapeutic potential of its modulation.

서 론

정상 성인의 장 내에는 사람의 총 세포수의 10배에 가까운 1013-1014개에 이르는 세균, 곰팡이, 원생동물 등 다양한 미생물이 군집을 이루고 있다[1]. 장내 미생물 무리는 대략 500-1,000종이 존재하며 인간 유전체의 100배 이상을 가지고 있다. 장내에는 주로 4개의 문(Bacteroidetes, Firmicutes, Actinobacteria, Proteobacteria)이 있는데 BacteroidetesFirmicutes 문이 장내 세균의 70-90% 이상을 차지한다[2]. 장내 미세 환경은 면역체계 형성과 발달에 중요한 역할을 하고 특히, 장내 미생물 무리는 숙주의 면역체계 조절에 중요한 역할을 하는 것으로 알려져 있다[2,3]. 최근 장내 세균이 숙주의 에너지 균형과 대사기능에 영향을 준다는 증거들이 보고되면서 비만과 장내 세균에 대한 연관성을 확인하고자 하는 연구들이 많이 이루어지고 있다. 본고에서는 최근까지 이루어진 연구들을 바탕으로 장내 미생물 무리가 비만의 병태생리에 관여하는 기전들을 확인하고 이를 바탕으로 향후 비만의 치료에 적용될 수 있는 가능성을 고찰해보고자 한다.

본 론

장내 미생물 무리가 비만에 주는 영향과 기전

장내 세균 종류의 변화

비만과 장내 미생물 무리의 변화가 연관이 있다는 최초의 증거로 발표한 Ley 등[4]의 연구에 따르면, 유전자 조작을 통해 만든 비만 쥐 모델에서 비만할수록 장내 미생물 무리 중 상대적으로 Firmicutes가 증가되고 Bacteroidetes가 감소되어 있다고 최초로 보고하였다. 이후 Turnbaugh [5]와 Turnbaugh와 Gordon [6]의 연구에 따르면, 마른 사람에 비하여 비만한 사람에서 장내 세균 중 Firmicutes 문의 비율이 높은 반면 Bacteroidetes 문 비율은 감소하였다. 또한 비만한 사람이 저탄수화물 혹은 저지방식이를 통하여 1년 이상의 체중 감소를 하는 경우, FirmicutesBacteroidetes의 비율이 마른 사람의 것과 점점 유사해진다[7]. 최근 비만 치료에 적용하는 Roux-en-Y gastric bypass 수술을 받은 환자들을 대상으로 한 연구에서 수술 후 대사 기능이 개선된다는 보고가 있고 Osto 등[8]은 수술 후 장내 세균의 균주의 변화를 보고하였다. 이러한 연구 결과들은 장내 세균 무리 종류의 변화가 비만의 병인으로 고려될 수 있는 근거가 된다.

장내 미생물 불균형(dysbiosis)

장내 미생물의 주된 역할은 사람이 정상적으로는 소화 흡수할 수 없는 다당류(resistant starch, oligosaccharides, inulin 등 polysaccharides)를 발효시켜 단쇄지방산(short chain fatty acids, SCFA)을 만들어 에너지원을 공급하는 것이다[9]. 소장에서 소화되지 않은 탄수화물로부터 장내 미생물 무리에 의하여 만들어진 SCFA는 인체에 흡수되어 숙주에게 에너지를 제공한다. 따라서 장내 미생물 무리는 음식으로부터의 ‘에너지 수확(energy harvest)’에 기여한다고 할 수 있다. 장내 미생물 무리는 체질량지수 30 kg/m2 이상인 비만 환자군에서 정상인에 비하여 SCFA 종도가 높게 측정되었고[10], SCFA의 한 종류인 acetate가 대뇌 포만 중추에 신호를 전달하는 역할을 한다[11]. 고지방 식이를 하면 장내 미생물이 경도 염증을 유발함으로써 비만을 유발할 수 있다는 근거들이 제시되고 있다. 생쥐에게 고지방 식이를 주었을 때, 장의 투과성이 증가하여 미생물의 외막 성분인 lipopolysaccharide (LPS)가 혈중에서 증가하고 LPS의 증가는 비만, 경도 염증과 인슐린저항을 유발하였다[12]. 또한 고지방 식이를 하면 염증을 유발하는 사이토카인(pro-inflammatory cytokine)이 증가하여 경도 염증이 지속적으로 유발되며 이러한 것이 인슐린 저항성과 비만과 같은 대사이상의 원인이 된다[13]. 그람 음성 세균의 세포벽에는 패혈증(endotoxemia)을 유발하는 지질다당체(lipopolysaccharide, LPS)로 구성되어 있는데 고지방 식이를 하면 LPS가 더욱 증가되어 다양한 조직에서 염증성 사이토카인의 생산을 자극한다[14]. 고지방 식이를 하게 되면 LPS를 함유한 그람 음성 세균이 장내에서 증가되고 장내세포를 연결하는 tight junction ZO (zonula occludens)-1과 occludin (zonulin)의 발현이 감소하면서 장 투과성이 증가되며[15], 이러한 결과는 장 투과성이 증가되면 혈청 LPS 증가로 인하여 경도 염증이 발생하며 인슐린 저항성과 같은 대사이상이 나타나 비만을 유발한다는 근거를 제시해 준다. 이처럼 장내 미생물의 불균형은 SCFA의 생성속도를 변화시켜 비만 등과 같은 특정 질환의 발현과 연관될 수 있으며 장관의 경도 염증 유발과 장막투과성 증가 등을 일으켜 비만의 병태 생리에 관여하는 것으로 생각된다.
이 외에도 특정 미생물 등과 비만의 연관관계가 보고되었다. 특히 Akkermansia muciniphila는 비만과 대사 질환을 조절하고 예방하는 데 주요 역할을 하는 미생물 중 하나이다[16]. Leptin 결핍 비만 쥐의 장내 미생물총에서 A. muciniphila의 감소, 프리바이오틱스(oligofructose)의 사용으로 A. muciniphlia의 회복, LPS의 감소로 이어지는 대사적 내 독소혈증의 감소 등의 연구 결과를 토대로 A. muciniphilia가 증가될 때 대사적 내독소 혈증과 관련된 대사 질환들이 줄어들었다. 이 외에도 Lactobacillus는 대장균과 살모넬라균과 같은 장내 병원균의 활동성을 억제할 수 있고 항비만효과가 최근 보고되었으며[17] 이소플라본(isoflavone aglycones)을 발효시켜 지나친 지방의 흡수와 지방세포의 분화(3T3-L1 adipocyte)를 효과적으로 억제하였다[18].

장내 미생물 무리 조절과 비만 개선을 위한 시도

최근의 연구들에서 장내 미생물 무리를 조절하여 비만을 개선하기 위한 시도들이 제시되고 있지만 아직까지 직접적인 비만의 치료에 적용할 수 있는 결과는 부족하다. 장내 미생물 무리의 조절을 위한 시도는 매일 섭취하는 식이의 조절을 통한 방법부터 프리바이오틱스나 프로바이오틱스의 섭취와 분변이식을 통한 방법까지 다양하게 연구되고 있다(Fig. 1). 49명의 과체중 혹은 비만 환자를 대상으로 6주간 탄수화물 제한 식이를 진행한 MICRO-Obes 연구[19]에 따르면 식이 조절 후 장내 미생물 무리의 유전자 다양성이 관찰되었다. 프로바이오틱스는 살아있는 세균을 주입하여 장내 미생물의 변화가 일어나 숙주에게 유익한 영향을 미친다고 알려져 있다[20]. 메타연구에서 Lactobacillus fermentumLactobacillus ingluviei는 체중을 증가시켰고, Lactobacillus plantarumLactobacillus gasseri는 체중 감소 효과를 보였다[21]. 또한 정상적인 장 속에서 Bifidobacteria는 LPS의 농도를 낮추는 효과가 있다고 알려져 있으며, Bifidobacteria의 수는 체지방, 당불내성, LPS 수치와 반비례한다[22].
장내 미생물 무리 연구 중 분변 이식을 이용한 연구가 최근 활발하게 진행되고 있다. Ridaura 등[23]의 동물 연구에 따르면 비만한 공여자의 장내 미생물 무리를 분변 이식을 통하여 생쥐에게 주었을 때 체중이 증가하였다. 인간 대상의 연구에서도 마른 공여자의 분변을 대사 증후군이 있는 환자에게 이식하였을 때, 이식 후 6주까지 체질량지수의 감소는 현저하지 않았으나 인슐린 민감도가 증가하였다[24]. 이러한 결과로 분변 이식을 통한 장내 미생물 무리의 다양성(diversity) 증가와 변화가 대사 질환의 개선을 유도하였을 것으로 추측할 수 있다.

결 론

비만의 원인은 매우 다양하고 복합적이다. 단순 에너지 불균형에서 호르몬, 유전 등이 큰 요인이 되고 이에 관련된 많은 요인들이 관련될 수 있다. 장내 세균의 변화가 에너지 흡수에 영향을 주고 에너지에 관련된 효소를 조절하고 장내 세균의 불균형으로 인하여 발생되는 경도 염증과 장내 투과성의 변화가 발생하여 에너지 대사에 영향을 주는 것이 장내 미생물 무리 외 비만의 상호작용에 관여한다. 또한 특정 미생물들과 비만과의 연관성도 제시되고 있다. 비록 아직까지 기전이 완전히 밝혀지지 않은 상황이나, 장내 미생물 무리는 비만 발생의 매우 중요한 역할을 하는 것으로 생각되고 향후 좀 더 심도 있고 종합적인 기초와 임상 연구를 통하여 장내 미생물 무리의 조절을 통한 비만 치료에 의미 있는 발전이 있을 것으로 기대한다.
