2018 대한부정맥학회 심방세동 위험인자와 동반된 심혈관계 질환의 발견과 관리

Abstract

A number of concomitant conditions and cardiovascular diseases are closely related to the development of atrial fibrillation (AF), AF recurrence, and AF-associated complications. Detection, prevention, and treatment of such conditions are essential for the prevention of AF and its disease burden. This article discusses the clinical conditions and concomitant diseases associated with AF including heart failure, hypertension, diabetes, obesity, chronic respiratory diseases, and kidney disease based on the 2016 European Society of Cardiology guidelines for the management of AF and recently updated clinical data, particularly in patients with heart failure. Furthermore, we provide recommendations for the prevention, diagnosis, and management of these conditions.

서 론

많은 심혈관계 질환 및 동반된 상태들이 심방세동 발생 위험을 증가시키고(Table 1), 심방세동의 합병증과 재발에 관련된다. 이러한 상태를 찾아 치료하는 것이 심방세동을 예방하고 심방세동의 진행과 합병증 예방에 매우 중요하다[1,2].

심 부 전

많은 환자에서 심부전과 심방세동은 함께 발생한다[3-5]. 공통된 위험인자와 병태생리로 연결되어 있기 때문이다[6]. 심부전과 심방세동은 심장 구조적 재구도화(remodeling), 신경호르몬 메커니즘의 활성화 그리고 심박수와 연관된 좌심실 기능부전과 같은 기전을 통하여 서로를 야기하거나 악화시키기도 한다. 심부전이 있는 심방세동 환자들은 좌심실 수축 기능이 유지되든지(좌심실 박출률 ≥ 50%) 그렇지 않든지(좌심실 박출률 < 40%) [7,8]에 관계없이 모두 증상이 더 심하고 사망률도 더 증가한다[9,10]. 최근 유럽심장학회 심부전 가이드라인에 따르면[11] 좌심실 박출률이 40-49%인 환자를 “박출률 경계 심부전(heart failure with mid-range ejection fraction, HFmrEF; left ventricular ejection fraction 40-49%)”으로 새롭게 정의하였으나 심방세동 환자에서의 데이터는 부족한 실정이다. 합병증을 예방하고 생활의 질을 좋게 유지하는 것은 좌심실 박출률과 상관없이 모든 심부전이 있는 심방세동 환자의 치료 목표이다[12]. 심방세동의 일반적인 치료 방법은 심부전 여부에 따라 차이는 없지만 몇 가지 주목해야 할 점이 있다. 중요한 점은, 예후를 개선하는 것으로 증명된 유일한 치료는 항응고 치료이고, 환자의 위험도에 따른 적합한 항응고제가 처방되어야 한다.

박출률 저하 심부전(heart failure with reduced ejection fraction, HFrEF)이 동반된 심방세동 환자

항응고 치료에 더해서, 가이드라인에 따른 표준적인 심부전 치료가 선행되어야 한다[11]. 이는 안지오텐신전환효소억제제(angiotension converting enzyme inhibitor, ACEi)나 안지오텐신II수용체차단제(angiotension II receptor blocker, ARB), 염류코르티코이드수용체길항제(mineralocorticoid receptor antagonist), 삽입형 심장충격기, 심장 재동기화 치료를 포함하며[6] 안지오텐신전환효소억제제나 안지오텐신수용체차단제 복용에도 증상이 지속되는 환자에게 적용될 수 있는 안지오텐신II수용체차단제와 네프릴라이신억제제(angiotensin receptor and neprilysin inhibitor) 사용을 포함한다[13].
심박수 조절에 대하여 간단히 요약하면 베타차단제와 디곡신이 박출률 저하 심부전에서 적절하며 이 경우 베라파밀과 딜티아젬은 심근 수축력 저하 작용이 있어 선호되지 않는다. 베타차단제는 보통 임상적으로 안정적인 박출률 저하 심부전에서 첫 번째 치료 선택지이지만 메타분석에서는 위약군에 비하여 사망률의 개선을 보여주지는 못하였다[14]. 디곡신은 임상에서 흔히 처방되지만 심방세동 환자에서 직접적인 비교 임상 연구가 시행된 적이 없다. 관찰 연구들의 메타연구에서 디곡신은 심부전이 있는 심방세동 환자에서 사망률 개선에 중립적인 결과를 보였다[15]. 그러므로 박출률 저하 심부전이 있는 심방세동 환자에서의 심박수 조절은 환자의 증상과 특성에 맞추어야 한다. 급성심부전 악화의 경우 베타차단제 시작은 신중히 결정되어야 하며 신부전이 동반된 환자에서는 디곡신 부작용에 유의해야 한다.
박출률 저하 심부전이 동반되어 증상이 심한 심방세동 환자의 경우 심박수 조절에 추가로 동율동 전환이 필요할 수 있다. 빠른 심박수로 박출률 저하 심부전이 발생한 환자의 경우 동율동 전환이 선호된다[16-19]. 빈맥으로 인한 심근병증의 진단은 애매할 수 있어 동율동 전환 후 치료 반응으로 진단되는 경우도 있다[20]. 전극도자절제술은 유용할 수 있으나 최근까지 데이터가 부족했던 실정이나[16-19] 최근 새로운 연구 결과가 업데이트되었다. 그림 1에 심부전이 있는 심방세동 환자에 대한 접근법을 요약한다.

박출률 보전 심부전(heart failure with preserved ejection fraction, HFpEF)이 동반된 심방세동 환자

박출률 보전 심부전의 진단은 쉽지 않다. 이런 환자에서의 증상은 심방세동이 원인인지 심부전이 원인인지 규명하기 어렵기 때문이다. 심장 동율동 전환으로 감별해 볼 수 있겠으나 이러한 질병 상태와 예후를 개선시킬 수 있는 특이적인 치료는 현재 부족한 실정이다. 심장초음파검사에서 보이는 좌심실비후 및 이완기 기능부전이 증상이 있는 심방세동 환자에서 박출률 보전 심부전이 동반되어 있는지 감별하는 데 도움을 준다. 조직 도플러 검사에서 감소된 조기 이완기 심근 속도(e’)는 좌심실 이완 장애를 반영하며 조기 이완기 승모판 유입 최고 혈류속도(E)와 e’을 이용하여 계산되는 E/e’은 침습적으로 측정한 좌심실 충만압과 밀접한 연관성이 있다[21-25]. 나트륨이뇨펩타이드(natriuretic peptide) 수치 또한 박출률 보전 심부전을 진단하는데 도움을 주지만[11] 이 수치는 심방세동 환자에서 증가되어 있고, 적절한 진단적 기준에 대해서는 연구가 필요하다[26]. 심방세동과 박출률 보전 심부전이 함께 있는 환자의 경우, 고혈압이나 심근허혈 등 동반된 상태를 확인할 필요가 있으며 체액 평형을 조절하는 것이 주된 치료이다.

박출률 경계 심부전(HFmrEF)이 동반된 심방세동 환자

박출률 경계 심부전은 비교적 최근에 정립된 개념으로 심부전의 증상과 징후가 있고, 좌심실 박출률이 40-49%이고, 나트륨이뇨펩티드가 증가되어 있으며, 좌심실 비대, 좌심방 확장 및 이완기 기능 장애가 동반되어 있을 경우를 의미한다[11]. 그러나 심방세동 환자의 경우 심부전 유무와 상관없이 나트륨이뇨펩티드가 상승해 있고 좌심방 확장이 흔히 동반되기 때문에 진단하기 더욱 어렵다. 심방세동 자체가 좌심실 수축 기능을 저하시키고 심박동 주기도 가변적이기 때문에 좌심실 박출률 역시 가변적이고 평가하기 어렵다. 박출률 경계 심부전이 동반된 심방세동 환자의 특이 치료 지침이 마련되기 위해서는 추가적인 연구가 필요하다.

심부전 환자에서의 심방세동 예방

대규모 무작위 대조 연구들의 후향적 분석에 따르면 안지오텐신전환효소억제제 또는 안지오텐신수용체차단제는 심방세동의 발생 예방에 효과가 있다[27-29]. 그러나 안지오텐신전환효소억제제/안지오텐신수용체차단제에 의한 심방세동 발생 감소 효과는 박출률 보존 심부전 환자에서는 보다 덜 명확하며[30], 심부전이 없는 환자들에서는 관찰되지 않는다[31-33]. 안지오텐신II수용체차단제와 네프릴라이신억제제의 복합제 역시 앞서 언급한 두 약제의 결과와 큰 차이는 없는 것으로 보고되고 있다. 안지오텐신전환효소억제제/안지오텐신수용체차단제로 치료 중인 좌심실 박출률 저하 심부전 환자에서 베타차단제 치료는 심방세동 발생을 33% 감소시키는 것으로 알려졌고, 이는 동율동의 좌심실 박출률 저하 심부전 환자에서 베타차단제 치료의 중요성을 재차 강조하게 되는 결과라 할 수 있다[14]. 염류코르티코이드수용체길항제인 eplerenone 역시 좌심실 박출률 35% 이하의, NYHA Class II인 환자들에서 안지오텐신전환효소억제제/안지오텐신수용체차단제 및 베타차단제에 추가되었을 경우 심방세동 발생을 감소시킨다는 보고가 있다[34].

심부전 환자들에게서 심방세동의 고주파전극도자절제술 효과

최근까지 심방세동을 동반한 심부전 환자들에게 약물 치료를 이용한 리듬 조절이 맥박 조절보다 우월한 결과를 보였던 연구는 없다[35-38]. 그러나 고주파전극도자절제술을 이용한 심방세동의 치료는 약물 치료 대비 좋은 결과를 보인 연구들이 있다. 2016년에 발표된 AATAC (Ablation vs. Amiodarone for Treatment of Atrial Fibrillation in Patients with Congestive Heart Failure and an Implanted ICD/CRTD) 연구에서 심방세동의 고주파전극도자절제술은 좌심실 기능이상이 동반되어 있는 심방세동 환자들에게서 아미오다론(amiodarone)을 이용한 약물 치료보다 정상 동율동을 더 잘 유지하였고, 좌심실의 박출률을 향상시켰으며, 사망률과 심부전으로 인한 입원율을 의미 있게 줄인 바 있다[39]. 최근 심방세동을 동반한 선별된 심부전(좌심실 박출률이 35% 이하) 환자들을 대상으로 진행된 CASTLE-AF (catheter ablation versus standard conventional treatment in patients with left ventricular dysfunction and atrial fibrillation trial) 연구에서 고주파전극도자절제술을 시행한 그룹은 약물 치료 그룹(리듬 조절 혹은 맥박수 조절)에 비하여 전체 사망 위험이 47% 낮았고, 심혈관계 사망 위험은 51% 낮았다[40]. 그러나 3,013명의 대상 환자 중에 363명의 환자만을 선별하여 진행된 연구이며 이를 모든 심부전 환자들에게 적용하기에는 무리가 있다. 따라서 이에 대한 여러 후속 연구가 필요한 실정이다.

심부전이 있는 심방세동 환자에서의 고주파전극도자절제술 권고

첫 번째 추천 심부전의 증상이 있고 좌심실 기능이 35% 이하로 감소된 심방세동 환자들에게 적절한 심부전에 대한 약물 치료와 더불어 항부정맥 약물 치료로도 동율동의 유지가 되지 않는 경우는 고주파전극도자절제술을 고려할 수 있다.

고 혈 압

고혈압은 심방세동 환자에서 뇌졸중의 위험인자로 잘 알려져 있다[41,42]. 조절되지 않는 높은 혈압은 뇌졸중 및 출혈을 증가시킬 뿐 아니라 심방세동의 재발 위험도를 높일 수 있으므로, 적절한 혈압 조절은 심방세동 환자의 관리에 있어 핵심적인 부분을 차지한다고 할 수 있겠다[43]. 레닌-안지오텐신-알도스테론 체계의 억제가 심장의 구조적 재구도화(remodeling)와 심방세동의 재발을 예방할 수 있는 것으로 알려져 있으며[27,44], 최근의 대규모 인구집단을 대상으로 한 연구에서는 항고혈압약물 중에서 안지오텐신전환효소억제제 또는 안지오텐신II수용체길항제가 심방세동 발생을 예방하는 효과가 있었다는 보고가 있다[45]. 추가 분석에서는 이러한 약물들이 특히 심부전이나 좌심실 비대가 있는 환자군에서 심방세동 발생률을 낮추는 것으로 확인되었다[29,46]. 그러나 심부전 또는 좌심실 비대가 없는 환자군에서는 안지오텐신II수용체길항제가 심방세동의 발생이나 재발을 위약군에 비하여 유의하게 감소시키지는 못하였다는 연구들이 있어 해석에 유의가 필요하다[31,32]. 최근에 발표되었던 SPRINT 연구 및 2017년 미국 고혈압 진료 지침에서는 중증도 이상의 위험도를 가진 환자군에서는 엄격한 혈압 조절을 권장하고 있다[47,48]. 그러나 심방세동 환자의 혈압 목표에 대한 언급은 어느 연구 및 권고안에도 없는 상태로, 심방세동 환자에서 적절한 혈압 목표 설정에 대해서는 추가적인 연구가 필요하겠다.

심방세동 환자에서의 약제(안지오텐신전환효소억제제 또는 안지오텐신II수용체차단제) 사용 권고

첫 번째 추천 좌심실 박출률이 감소된 심부전 환자에서 심방세동의 발생의 일차적 예방을 위하여 안지오텐신전환효소억제제 또는 안지오텐신II수용체차단제를 고려해야 한다. (Class IIa, LOE B)
두 번째 추천 고혈압 환자에서 심방세동 발생의 일차적 예방을 위하여 안지오텐신전환효소억제제 또는 안지오텐신II수용체차단제를 고려할 수 있다. (Class IIb, LOE B)
추천 되지 않음 심혈관계 질환이 없는 환자에서 심방세동 발생의 일차적 예방을 위한 안지오텐신전환효소억제제 또는 안지오텐신II수용체차단제의 사용은 권고되지 않는다. (Class III, LOE B)

판막성 심장 질환

판막성 심질환은 심방세동의 발생과 큰 연관이 있다[49]. 대략적으로 30%의 심방세동 환자가 판막성 심질환을 가지고 있는 것으로 알려져 있으며, 주로 심장초음파검사로 확인된다[50-53]. 심방세동은 중증도 이상의 판막성 심질환을 가진환자의 예후를 악화시키는 것으로 알려져 있으며[54], 대동맥 판막 또는 승모판막 수술이나 경피적 중재술의 예후와도 큰 연관이 있는 것으로 보고된다[55-60]. 판막성 심질환은 심방세동 환자의 혈전 생성 및 색전 위험도, 그에 따른 뇌졸중 위험도를 증가시킨다[61]. 때로는 판막성 심질환에서 발생하는 심방세동이 판막성 심질환 자체가 진행되는 것을 알려주는 표지자로 인식되어, 심방세동의 발생 자체가 판막 성형술이나 치환술을 진행하는 적응증이 되기도 한다[62].
전통적으로 심방세동은 ‘판막성’과 ‘비판막성’ 심방세동으로 구분되어 왔다[63]. 권고안, 연구들에 따라 조금씩 다른 정의가 사용되고 있기는 하나, 일반적으로 ‘판막성’ 심방세동은 기계판막을 가지고 있는 경우와 류마티스성 판막 질환(주로 승모판협착증)을 가진 경우를 칭한다. 승모판협착증이 심방세동의 혈전색전증 및 뇌졸중 위험도를 증가시킨다는 사실은 잘 알려져 있으나[61,64,65], 그 외의 다른 판막성 심질환(즉, 승모판역류증, 대동맥판막협착증이나 역류증 등) 등이 심방세동의 뇌졸중 위험도에 영향을 준다는 증거는 현재까지 명확하지 않다[66].

판막성 심장 질환이 있는 심방세동 환자에서의 권고

첫 번째 추천 중등도 이상의 승모판역류가 있고 좌심실 수축 기능이 보전된 새로 발생한 심방세동 환자에서는 증상이 없다 하더라도 조기에 승모판막 수술(특히 판막 재건술이 가능할 경우)을 고려해야 한다. (Class IIa, LOE C)
두 번째 추천 증상이 없는 중등도 이상의 승모판협착 환자 중 판막 구조가 적절하고 새로 발생한 심방세동이 있을 경우 승모판 절개술을 고려해야 한다. (Class IIa, LOE C)

당 뇨

당뇨병과 심방세동은 공통 위험 요인이 있기 때문에 흔히 병발한다[67-73]. 또한 당뇨병은 심방세동 환자에서 뇌졸중의 위험인자이다[74]. 당뇨병 유병 기간이 길어질수록 혈전색전증의 위험도는 증가한다[75]. 불행히도 집중적인 혈당 조절이 심방세동의 새로운 발생률에 영향을 주지는 않았다[74]. 반면에 메포민 치료는 당뇨병 환자의 장기간 심방세동 위험률을 줄이는 것과 관련성이 있으며[76], 장기간 뇌졸중 위험을 낮춘다고 알려져 있다[77]. 질병의 중증도를 반영하는 당뇨망막병증은 항응고 치료를 받는 환자들의 안구출혈 위험을 증가시키지는 않는다[78].

비만과 체중 감소

위험 요인으로서의 비만

비만은 체질량지수에 따라 점진적으로 심방세동의 발생 위험을 증가시킨다[79-82]. 비만 환자에서는 좌심실 이완기 기능 장애, 증가된 교감신경 활동 및 염증 반응 그리고 심방내 지방 축적 등이 발생할 수 있다[83-85]. 또한 비만은 심방세동 환자에서 허혈성뇌졸중, 혈전색전증과 사망의 위험 요소가 될 수 있다[86]. 국내 연구에서는 비만이 심방세동의 지연성 재발에 관계한다는 연구와 서구인에 비하여 상대적으로 체질량지수가 작은 아시아인에서는 복부비만 자체도 새로운 심방세동 발생에 관련 있다는 보고가 있다[87,88].

심방세동이 있는 비만 환자에서의 체중 감소

다른 심혈관 위험 요소들의 관리 이외에도 집중적인 체중 감량(10-15 kg 체중 감량 범위 내)은 일반적인 관리에 비하여 심방세동의 재발을 감소시키고 증상을 호전시켰다[1,2,89]. 심폐운동의 향상은 비만한 심방세동 환자에서 심방세동의 발생 정도를 줄일 수 있다[90]. 이 연구 결과들은 대규모 연구에서 확인이 되어야 하지만, 비만한 심방세동 환자에서의 체중 감량이 긍정적 효과가 있을 수 있음을 뒷받침한다.

비만 환자에서 전극도자절제술

비만이 심방세동에 대한 전극도자절제술 후 심방세동의 재발률 증가와 관련이 있다는 보고가 있으며[91-94], 이는 폐쇄성 수면무호흡증이 주요한 교란변수로 작용한 영향으로 볼 수 있다. 또한 비만은 심방세동에 대한 전극도자절제술시 발생하는 누적 방사선량의 증가와 관련 있고[95], 시술 관련 합병증 발생률의 증가와도 관련이 있다는 보고도 있다[96]. 하지만 비만과 관련한 합병증 증가는 체질량지수가 40 이상인 심한 고도 비만이 동반된 환자에 국한 되었으며[96], 많은 연구에서 비만이 직접적으로 심방세동전극도자절제술의 심각한 합병증(사망, 뇌졸중, 심장 눌림증) 증가와 관련이 없었기 때문에[95,97-99] 중등도 및 그 이하의 비만 환자에서 시술 안정성은 크게 차이가 없는 것으로 보인다. 또한 비만인 환자에서 실시한 전극도자절제술의 결과, 삶의 질 향상 측면에서 비만하지 않은 환자의 결과와 비교하여 열등하지 않았기 때문에[97] 심방세동의 전극도자절제술은 필요한 경우라면 비만 환자에서도 적극적으로 시행되어야 한다. 또한 전극도자절제술을 시행한 환자에서 체중 감량을 포함한 다각적인 심혈관 위험인자 관리가 전극도자절제술 후 심방세동 재발을 의미 있게 낮추었다고 보고하였고[1], 체중 감량이 심방세동 발생빈도를 낮출 수 있기 때문에, 심방세동에 대한 전극도자절제술을 시행받은 비만 환자에서는 체중 감량을 위하여 적극적인 생활습관 개선도 함께 이루어져야 하겠다.

심방세동이 있는 비만 환자를 위한 권고

첫 번째 추천 다른 위험 요소와 함께 비만 환자에게서 체중 감량은 심방세동의 발생 정도와 증상 완화를 위하여 고려되어야 한다. (Class IIa, Level of B)

만성폐쇄성폐질환, 수면무호흡 및 호흡기 질환

심방세동은 폐쇄성 수면무호흡증과 관련이 있다[100,101]. 자율신경 이상, 저산소증, 고이산화탄소혈증, 염증 반응 등 여러 다양한 병리학적 기전이 심방세동 발생에 기여한다[100-104]. 폐쇄성 수면무호흡증은 흉곽내 압력 변화를 증가시켜 이로 인하여 직접적으로나 혹은 미주신경 항진을 통하여 심방 근육 세포의 활성 전위 시간을 감소시켜 심방세동을 유발할 수 있다. 수면무호흡증의 위험인자를 치료하고, 수면무호흡 증에 대하여 지속적인 양압기도환기 치료를 하는 것은 심방세동의 재발을 줄일 수 있다[105-109]. 수면무호흡증의 위험 요소를 동반하고 있는 심방세동 환자들에게서 수면무호흡증에 대하여 선별 검사를 하는 것이 추천된다. 또한 심방세동의 치료 효과를 향상시키기 위하여 폐쇄성 수면무호흡증에 대한 치료를 병행해야 한다. Servo-controlled pressure support 치료는 중추성 수면무호흡증이 동반되어 있는 좌심실박출률이 저하된 심부전 환자들에게 시행해서는 안 된다[110].
만성폐쇄성폐질환을 가지고 있는 환자들은 종종 심방 빈맥을 보이며, 심전도로 심방세동과 감별진단을 하는 것이 필요하다. 기관지 수축을 완화시키는 theophylline과 베타아드레날린 작용제(beta-adrenergic agonists)도 심방세동의 발생을 촉진시키고 심실 반응속도 조절을 어렵게 한다. 비선택적 베타차단제, sotalol, propafenone, adenosine은 기관지수축이 있는 환자들에게 조심스럽게 사용해야 하나, 만성폐쇄성폐질환을 가지고 있는 환자들에게는 비교적 안전하게 사용할 수 있다. 베타-1 선택적차단제(bisoprolol, metoprolol, nebivolol) 및 딜티아젬, 베라파밀은 기관지수축/경련이 있는 환자들에게서 큰 부작용 없이 사용할 수 있다.

만성폐쇄성폐질환, 수면무호흡 및 호흡기 질환이 있는 환자에서의 권고

첫 번째 추천 급성 호흡기 질환이 있는 환자에게 심방세동이 발생하였을 때 우선적으로 저산소증과 산혈증을 교정하는 것을 고려해야 한다. (Class IIa, LOE C)
두 번째 추천 심방세동 환자들에게서 폐쇄성무호흡증의 임상적 증후가 있는지 확인해야 한다. (Class IIa, LOE B)
세 번째 추천 심방세동 치료의 향상과 심방세동의 재발을 막기 위하여 폐쇄성무호흡증의 치료를 함께 병행해야 한다. (Class IIa, LOE B)

만성 신질환

만성 신질환 환자들 중 약 15-20%에서 심방세동이 병발된다[111]. 만성 신질환의 정의는 많은 연구에서 크레아티닌 청소율(creatinine clearance, CrCl)이 60 mL/min 미만일 때로 정의하지만 비-비타민 K 길항 경구용 항응고제 연구에서는 대부분 크레아티닌 청소율이 50 mL/min 미만일 때 비-비타민 K 길항 경구용 항응고제의 용량 조절을 권고하였다. 크레아티닌 청소율은 심방세동 환자들에게서 점차 감소할 수 있음이 관찰되었다[112].

만성 신질환 환자에서의 권고

첫 번째 추천 모든 심방세동 환자에서 함께 동반되어 있는 신질환의 진단 및 심방세동 약물 치료시 적절한 용량 조절을 위해서 혈청 크레아티닌 농도와 크레아티닌 청소율을 구하는 것이 필요하다. (Class I, LOE A)
두 번째 추천 항응고 치료를 받는 모든 심방세동 환자들은 적어도 1년에 한 번은 신기능을 평가해야 한다. (Class IIa, LOE B)
