Abstract

Rhythm control therapy is the main strategy for restoring and maintaining sinus rhythm in patients with non-valvular atrial fibrillation (NVAF). Sinus rhythm is better restored and maintained with antiarrhythmic drugs than with placebo treatment. In addition, catheter ablation or combination therapy is more effective than antiarrhythmic drugs for treating NVAF. However, in most clinical trials to date, rhythm control therapy has resulted in neutral clinical outcomes compared with rate control therapy. The decision to undergo rhythm control therapy should be based on age, atrial fibrillation (AF)-related symptoms, type of AF, structural heart disease, and underlying comorbidities. For now, rhythm control therapy is indicated to improve symptoms in patients with NVAF who have refractory symptoms after adequate rate control therapy. The Korean Heart Rhythm Society organized the Korean AF Management Guideline Committee and analyzed all available data, including South Korean patients with NVAF. This review article provides general principles and detailed methodology for rhythm control therapy in South Korean patients with NVAF.

서 론

심방세동 치료에 있어서 동율동으로 회복된 후에도, 지속적으로 동율동을 유지하는 것은 장기적으로 중요한 부분이다. 현재까지의 항부정맥제는 대조군에 비해서 약 2배 정도 효과적으로 동율동을 유지할 수 있다고 알려져 있으며[1-5], 추가적으로 항부정맥제를 사용하여 동율동 회복에 실패하였더라도, 도자절제술을 단독으로 하거나 약물과 도자절제술을 동반한 동반 치료를 하는 것이 훨씬 더 효과적이라고 알려져 있다[6-8]. 많은 임상의사들이 동율동으로 유지하는 것이 심방세동의 예후를 개선시킬 수 있다고 생각하였지만[9], 현재까지 리듬 조절 전략과 맥박수 조절 전략을 비교하는 모든 연구들에서 (적절한 항응고제 사용을 포함) 비슷한 결과를 보여주었다[3,10-16]. 최근 리듬 조절 전략으로 약물 동반 혹은 초기에 단독으로 도자절제술을 시행하는 전략이 주요한 심혈관 및 뇌혈관 질환 발생률을 감소에 관한 전향적 비교 연구가 진행 중에 있다(EAST-AFNET4와 CABANA) [17,18]. 현재 심방세동 치료에 있어서 도자절제술은 적절한 리듬 조절 치료에도 불구하고 증상이 지속되는 심방세동 환자에서, 여러 증상들을 개선시키기 위한 목적으로 선택될 수 있다.

급성기 동율동 조절 치료

동율동의 회복을 위한 항부정맥제 치료

항부정맥제는 심방세동 상태를 동율동으로 전환시킬 수 있다는 것을 (약물에 의한 심장율동 전환) 작은 규모의 연구들 혹은 메타분석 연구에서 보여줬다[13,19-29]. 전기적 심장 율동 전환은 약물에 의한 심장율동 전환에 비해서 짧은 기간 동안 훨씬 더 빠르고, 더 효과적으로 심방세동에서 동율동으로 회복시킬 수 있으며, 입원 기간도 더 짧게 할 수 있다[30-34]. 하지만, 이와는 달리 약물에 의한 심장율동 회복은 전기적 심장율동 전환과는 달리 진정 및 수면이나 금식들의 조치들이 필요치 않다.
플레카이니드(flecanide)와 프로파페논(propafenone)은 대표적인 항부정맥제로서 약물에 의한 심장율동 전환에 있어서 효과적으로 사용될 수 있으나[26-29,35,36], 구조적 심질환이 없는 환자에게만 제한적으로 사용될 수 있다. 그 다음으로 사용되고 있는 항부정맥제인 아미오다론(amiodarone)은 심부전 및 허혈성 심장 질환을 가지고 있는 환자에게 일반적으로 사용될 수 있으며[20], 정맥주사 이후 8-12시간 안에 맥박수를 분당 10-12회 정도를 감소시킬 수 있다(Table 1) [20]. 아미오다론과 플레카이니드는 모두 동율동 회복률에 있어서 소타롤(sotalol)보다 더 효과적으로 알려져 있다[24,25,37,38].

환자에 의해서 시행될 수 있는 ‘pill in the pocket’ 치료

발작성 심방세동으로 인해 간헐적으로 발작적인 증상을 경험하는 일부 환자들에 있어서는, 병원 안에서 경구용으로 일회 사용 용량인 플레카이니드 200-300 mg 혹은 프로파페논 450-600 mg이 안전하게 사용되었음을 확인하였다면, 병원 밖에서 경구용 플레카이니드 200-300 mg 혹은 프로파페논 450-600 mg을 간헐적으로 발생하는 발작적인 증상 발생시에 자가로 투여함으로써, 동율동으로 회복시키기 위한 치료로 사용될 수 있다[39]. 이런 치료법은 병원 안에서 시행되는 심장율동 전환보다 효과적이지는 못하지만[40], 일부 환자들에게는 실질적인 도움을 줄 수도 있다.

전기적 심장율동 전환

동기화된 직류전기를 통한 전기적 심장율동 전환은 빠르고 효과적으로 심방세동을 동율동으로 전환시킬 수 있으며, 최근 발생한 심방세동으로 인하여 혈역학적으로 불안정한 환자에게 유일하게 선택할 수 있는 치료법이다[41-43] (Fig. 1).
이런 전기적 심장율동 전환은 미다졸람 혹은 프로포폴 정맥주사를 통해서 진정 및 마취된 환자들에게서 안전하게 시행될 수 있다. 심장율동 전환 치료 동안 지속적으로 혈압 및 산소포화도를 감시하는 것이 중요하며[44], 치료 이후 간혹 환자에게서 전기 패취 부위에 피부화상이 관찰되는 경우도 있기 때문에 이를 관찰하는 것도 필요하다. 아트로핀 혹은 이소프레테레놀 정맥주사 및 체피형 임시 박동은 심장율동 전환 이후에 발생할 수 있는 심한 서맥성 부정맥을 방지할 수 있다. 양방향 세동파형 제거법은 단방향 파형보다 더 효과적이어서 현재 표본이 되어있으며[41,42], 신체 가슴 앞쪽과 뒤쪽에 전기 패취를 위치시키는 것이, 신체 가슴 앞쪽과 왼쪽 옆쪽으로 위치시키는 것보다 좌심방에 보다 더 강력한 전기적 충격장을 발생시켜, 더 효과적으로 동율동을 회복시킬 수 있다[42,43,45].
전처지로 투여하는 아미오다론(경우에 따라서 몇 주가 필요할 수도 있다) [46,47], 소타롤[46], 플레카이니드[5] 및 프로파페논[48]은 향후 시행하는 전기적 심장율동 전환의 효율을 증가시킬 수 있다. 하지만 전처치로 투여하는 베타차단제, 베라파밀, 딜티아젬[49-51] 및 디곡신[52,53]은 동율동으로 회복시키거나, 전기적 심장율동 전환 효율을 증대시키는 효과는 없는 것으로 알려져 있다. 전기적 심장율동 전환 이후에 동율동을 효과적으로 유지하기 위해서는, 전기적 심장 율동 전환 1-3일 전(아미오다론은 몇 주 전이 될 수 있다)에 항부정맥제를 전처치로 투여하여 약물의 농도를 유지한 상태에서 시행한 전기적 심장율동 전환이 동율동 유지 효과를 증대시킬 수 있다[5,25].

심장율동 전환을 시행한 환자에 있어서 항응고 요법

항응고 요법을 하지 않는 심장율동 전환은 뇌졸중 발생의 위험을 가지고 있으며[54], 항응고 요법에 의해서 상당부분 이런 뇌졸중 발생 위험이 감소된다[54]. 즉각적인 항응고 요법의 시작은 심장율동 전환을 예정하고 있는 모든 환자에게 중요하며[55-58], 48시간 이상 동안 발생한 심방세동을 가지고 있는 환자가 심장율동 전환을 계획하였으면, 경구용 항응고 요법을 3주 이전에 시작하고, 동시에 심장율동 전환 치료 이후 4주간 동안 항응고 요법을 시행해야 된다(지속적으로 항응고 요법이 필요하지 않는 환자의 경우). 만약, 뇌졸중 발생 위험이 있는 기저 질환을 가지고 있는 환자라면, 경구용 항응고 요법을 영구적으로 시행해야 한다[59]. 이러한 치료 방침은 무작위 대조 연구에서 증명된 바는 없지만 핀란드에서 시행된 대규모 관찰 연구에서 안전한 것으로 확인되었다[60]. 현재 진행 중인 연구는 심장율동 전환을 계획하고 있는 환자에 있어서 NOAC을 사용한 항응고 요법의 안정성과 효율성에 대해서 정보를 제공하고 있다.

장기적 동율동 조절 치료

항부정맥제 사용의 목적은 심방세동과 관련된 증상을 호전시키는 것이다[13]. 따라서 장기간의 항부정맥제 치료를 시작할 때는 증상의 정도, 약제의 부작용, 환자의 선호도 간의 균형을 잘 맞추어야 한다. 항부정맥제를 사용할 때의 원칙은 다음과 같다: 1) 치료의 목적은 심방세동과 연관된 증상의 감소이다. 2) 정상 동율동을 유지하는 항부정맥제의 효과는 중등도 정도이다. 3) 임상적으로 성공적인 항부정맥제 치료는 심방세동을 없애기 보다는 감소시킨다. 4) 한 가지 항부정맥제가 실패하였을 때는 다른 약제를 사용하여 임상적으로 만족할 만한 반응을 얻는 경우도 있다. 5) 약제에 의한 부정맥의 유발 가능성 또는 심장 외의 부작용이 흔하다. 6) 효과보다는 안전성에 대한 고려가 항부정맥제의 선택에서 우선시 되어야 한다.
항부정맥제를 사용하면 사용하지 않았을 때보다 정상 동율동이 유지될 확률이 약 2배 정도 높으나[61], 사망률이나 심혈관계 합병증을 증가시키는 효과는 없고 율동 조절 치료는 항부정맥제의 부작으로 인해 입원을 경미하게 증가시킬 수 있다[3,10-14]. 그러므로 항부정맥제의 부작용을 줄이기 위해 항부정맥제의 사용 기간을 단축시키는 것이 바람직하다. 예를 들어, 전기적 심율동 전환 치료 후에 플레카이니드를 이용한 4주간의 단기간 치료는 장기간 치료와 비교할 때 부작용이 적고 대부분의 심방세동 재발을 예방하였으며[5], 단기간의 항부정맥제 사용은 심방세동의 전극도자절제술 이후 조기재발을 예방하기 위해서도 사용된다[62]. 항부정맥제의 단기간 사용은 부작용의 위험성이 높은 환자나 심방세동이 재발할 가능성이 낮은 환자에게도 합리적이다. 항부정맥제와 전극도자절제술 외에 체중 감량, 혈압 조절, 심부전 치료 등 동반된 심혈관 질환의 치료도 정상 동율동을 유지하기 위해 도움이 된다.

장기적 치료를 위한 항부정맥제의 선택: 안정성 우선

일반적으로 항부정맥제를 처음 선택할 때는 약제의 안전성을 먼저 고려하여야 한다(Fig. 2). 심방세동의 발생을 막기 위해 다음과 같은 항부정맥제들이 사용 가능하다(Table 2).

아미오다론

아미오다론은 다양한 이온채널을 차단하여 효과를 나타내며 사용가능한 항부정맥제 중 가장 효과가 좋은 약제이다. 심박수를 줄여주어 다른 맥박수 조절을 위한 약제가 필요 없게 되는 경우가 많으며 심부전, 좌심실 비대, 관상동맥 질환 환자에게도 안전하게 사용할 수 있다[3,63]. torsades de pointes와 같은 부정맥을 일으킬 수 있으므로 심전도상에서 QT 간격과 T 및 U파를 주의 깊게 관찰해야 한다[64]. 아미오다론은 특히, 장기적으로 사용할 때 갑상선, 간, 폐, 안구, 피부 등에 부작용을 일으킬 수 있어 주기적인 검사가 필요하다. 가장 치명적인 경우인 폐독성과 같은 부작용이 용량에 의존적이므로 유지용량을 가능한 하루 200 mg 이하로 사용하는 것이 바람직하고 다른 항부정맥제를 사용할 수 있는 경우라면 2차 약제로 사용하는 것이 권장된다[65]. 아미오다론은 반감기가 수주로 길기 때문에 단기간 치료 목적으로 사용하는 것은 적절하지 않다[66].

드로네다론

드로네다론은 아미오다론과 구조가 비슷하나 요오드를 포함하고 있지 않아 아미오다론의 부작용을 감소시킨 약제인데 효과는 아미오다론에 비해 떨어진다. 드로네다론은 적어도 하나 이상의 심혈관 질병을 가지고 있는 발작성 또는 지속성 심방세동 환자에서 동율동을 유지하고 심박수를 줄여주며 심혈관계로 인한 입원을 줄여주고 심혈관 사망을 감소시키는 것으로 보고되었다[4,9]. 하지만 비대상성 심부전(심방세동 유무와 상관 없이) 환자와 영구적 심방세동 환자에서 사망률을 증가시키는 것으로 나타나 NYHA III 또는 IV 심부전과 동율동이 회복되지 않은 상태에서는 금기이다[67,68]. 주요 부작용은 서맥과 QT 간격의 연장이며 크레아티닌 수치를 증등도로 증가시키는데 이것은 신장 기능의 감소라기보다는 배설의 감소로 인한 것이다[69].

플레카이니드, 프로파페논, 필시카이니드

플레카이니드, 프로파페논 및 필시카이니드는 Vaughan Williams class IC 약제로 구조적인 심질환이 없는 심방세동 환자에서 심율동 조절을 위해 사용될 수 있다. 다른 나트륨 통로 차단제와 마찬가지로 플레카이니드는 심근경색이 있었던 환자에서 사망률을 상승시킬 수 있으므로 허혈성 심질환이 있는 환자에서 사용을 피해야 하며[70,71], 같은 계열 약제인 프로파페논과 필시카이니드 또한 심근경색이 있었던 환자에서 사용을 피해야 한다. 추가적으로 세 가지 약제 모두 심근 수축력을 감소시킬 수 있으므로 좌심실부전 환자에서도 사용을 피해야 한다. 또한, 세 약제 모두 심방조동 동안에 심방 박동수를 감소시켜서, 1:1 방실전도를 초래하여 심실 박동수를 증가시킬 수 있다. 따라서 방실전도 차단약제를 반드시 같이 사용해야 한다. 약제 사용에 따른, 사용-의존적(use-dependent) PR 및 QRS 간격의 증가가 약제 사용 전과 비교하여 25%까지 관찰될 수 있다. QRS 간격이 25% 이상 증가하는 것은 약제 사용과 연관된 부정맥 발생과 연관이 있다[72]. 그러므로 심실내 전도 장애 또는 각차단을 포함하여 전도 장애가 있는 환자의 경우, 특히 심박동기가 없는 경우에서는 주의해서 약제를 사용을 해야 한다[73]. 심장 이외의 부작용은 흔하지 않으나, 어지럼증, 시야 장애가 발생할 수 있으며, 프로파페논의 경우 미각 이상을 초래할 수도 있다[74,75]. 프로파페논은 베타 차단 효과가 있으며, 플레카이니드와 함께 CYP2D6의 기질이 되며, 이것은 7% 인구에서는 없는 것(poor metabolizer)으로 알려져 있다. CYP2D6은 퀴니딘, 플루옥세틴 및 삼환계 항우울제로 억제된다. 따라서 약제 상호작용 또는 유전적 취약성이 있으면 높은 혈중 농도를 초래할 수 있다. 플레카이니드와 프로파페논은 발작성 심방세동뿐만 아니라[39,76], 지속성 심방세동 환자에서도 심방세동 재발 방지 또는 동율동 유지 기간 연장에 도움이 된다[5,70,77-81]. 총 21,305명이 포함된 59개의 연구를 이용한 최근의 메타분석에 따르면, class IA 약제 및 소타롤은 모든 원인 사망률의 증가와 연관이 있으나 플레카이니드와 프로파페논은 사망률의 증가와 관련이 없다고 보고되었다[61]. 특히 class IC 약제는 심방세동의 재발률을 상당히 감소시킨다고 보고되었다. 플레카이니드와 프로파레논은 드로네다론 및 소타롤과 비교하여 심방세동 재발 감소 효과가 비슷하며, 아미오다론보다는 심방세동 재발 감소 효과가 약하다[82,83]. 필시카이니드는 일본에서 개발된 class IC 약제로 현재 우리나라에서도 경구제제로서 사용되고 있다. 심방세동의 동율동 전환 및 유지 모두에 효과적이며[84] 최근의 한 교차 연구에서 필시카이니드가 발작성 심방세동 환자의 재발률과 삶의 질에 있어서 플레카이니드와 차이가 없는 것으로 보고되었다[85].

소타롤

소타롤은 Vaughan Williams class III 약제로서 회귀성 기전(re-entry mechanism)을 가진 부정맥을 억제하는 데 효과적인 약물이다. 소타롤은 동율동 전환에는 효과가 없으나, 일단 동율동 전환이 된 환자에서 심방세동의 재발 방지에는 효과적이다. 심방세동 환자의 30-50%의 환자에서 1년간 동율동이 유지되는 것으로 알려져 있다[86,87]. Canadian Trial of Atrial Fibrillation Investigations 연구에서, 16개월의 관찰 기간 동안 아미오다론 복용군에서는 35%의 환자에서 재발하였지만 소타롤 복용군에서는 63%의 환자에서 재발하였다[2]. 동율동 유지 효과를 비교한 다른 연구(Sotalol amiodarone atrial fibrillation efficacy trial [SAFE-T] 연구)에서도 소타롤이 아미오다론에 비하여 효과가 적은 것을 확인하였으나[25], 프로파페논과는 효과가 비슷한 것으로 확인되었다[2]. SAFE-T 연구에서는 허혈성 심질환을 가진 환자군에서 소타롤 및 아미오다론을 복용하는 경우 허혈성 질환이 없는 환자에 비하여 동율동 유지가 보다 효과적인 것으로 확인되었다[25]. 또한 이 연구에서는 또한 허혈성 심질환을 가진 환자에서 아미오다론과 소타롤이 동율동 유지 효과가 비슷한 것으로 확인되었다[25]. 최근 수행된 연구에서는, 심방세동 및 관상동맥 질환을 함께 가지고 있는 환자에서 소타롤을 복용한 군이 아미오다론을 복용한 군에 비하여 생존율이 높았으며, 부작용도 적어 약물에 대한 순응도도 좋았다고 보고하였다[88]. 따라서 심방세동 환자에서 허혈성 심질환이 동반될 경우, 아미오다론보다는 소타롤을 먼저 선택하는 것이 적절할 수 있다. 소타롤은 약물 유발성 긴 QT 증후군을 잘 일으키는 것으로 알려져 있으며, 일부 비교 연구에서는 소타롤 사용군에서 사망률이 증가하는 경향(odds ratio 3.44, 95% confidence interval 1.02-11.59)도 확인되었다[82]. 소타롤은 간에서 거의 대사되지 않고 신장에서 배설되므로, 만성 콩팥병이 있거나 콩팥기능이 불안정한 환자에서 약물 유발성 긴 QT 증후군의 발생 여부에 주의를 기울여야 한다[89]. 소타롤을 안전하게 사용하기 위해서는, 심전도 상에서 QTc가 450 ms 이하일 경우 약물을 시작하여야 한다. 또한 최소한 3일간 심전도를 시행하여, QT 구간이 500 ms 이상으로 증가할 경우 약물의 용량을 감량하여야 한다. 한편, 소타롤은 활동 전위 기간(action potential duration)을 증가시키는데, 이는 맥박수가 감소할수록 QT 간격이 길어지게 되며, ‘reverser use dependence’라는 현상을 유발한다[90]. 따라서 소타롤을 복용하는 환자에서 서맥이 발생할 경우 서맥으로 인한 torsades de pointes가 발생할 수 있다는 점을 유념하여야 한다.

부정맥 유발 위험성을 구별하기 위한 12유도 심전도 소견

심방세동 치료 목적으로 항부정맥제 복용시, 심장내 전기생리학적 변화가 나타나게 되므로[91], 오히려 부정맥을 유발시킬 수 있어 주의를 요한다. 고위험군을 예측할 수 있는 소견은 12유도 심전도의 변화로부터 감지할 수 있기 때문에 항부정맥제 처방 전 반드시 기저 심전도를 미리 기록해두는 것이 필요하다. 만일 기저 심전도에서 심각한 동기능 부전 혹은 전도 장애가 있거나 QT 간격이 500 ms 이상으로 이미 길어져 있으나 심박동기가 없는 환자에서는 항부정맥제 처방시 더욱 주의가 필요하다[73]. 약제 사용 중에는 심전도상 PR [72,92], QT [93] 및 QRS [72] 간격, U파의 발생[64] 등이 중요한 변화 소견인데, Vaughan Williams class IC 약제인 플레카이니드, 프로파페논 및 필시카이니드는 주로 나트륨 통로를 억제시켜 심장내 전도 속도를 늦추므로 PR 및 QRS 간격이 연장된다[94]. 이는 정상적인 약제 반응이지만 만일 기저 심전도에 비해 간격이 25% 이상 증가하게 되면 약제 감량 또는 중단이 필요하다[72,95]. Vaughan Williams class III 약제인 소타롤, 드로네다론 및 아미오다론은 주로 칼륨 통로를 억제하므로 재분극 이상이 두드러지게 나타난다[94]. 즉, 이러한 약제 복용 중 QT 간격이 500 ms 이상 또는 기저 심전도보다 60 ms 이상 증가하거나 새로운 U파 발생시 Torsades de pointes 발생의 위험신호이므로 항부정맥제 감량 또는 중단을 고려해야 한다[64,96-98]. 따라서 항부정맥제를 복용하는 환자는 심전도의 주기적 추적 관찰이 필수적인데, 약제 사용 초기에 심전도 변화가 가장 뚜렷하므로 class IC 약제 및 소타롤은 약제 시작 3일째까지 매일, amiodarone은 1주 및 4주 후, dronedarone은 1주 후 확인이 필요하며, 이후로도 모든 약제에서 최소 6개월에 한 번씩의 심전도 추적 관찰이 권고된다[5,97,99].

새로운 항부정맥제

이상에 언급된 기존에 상용되고 있는 항부정맥제 이외에도 다양한 이온 채널을 억제하여 항부정맥 효과를 나타내는 약제들이 있다.

베르나칼란트(vernakalant)

다른 항부정맥제에 비해 심방 선택적이며 초기 활성 칼륨 통로 및 박동수 의존 나트륨 통로를 선택적으로 차단하며 사용-의존성(use-dependency)을 보인다는 점에서 특이한 약제이다[26,100]. 구조적 심질환이 없거나 심하지 않은 환자에서 3시간-7일내 급성으로 발생한 비교적 혈역학적으로 안정된 심방세동의 빠른 동율동 전환을 기대해볼 수 있는 정맥주사제로, 위약과 비교한 3상 연구나 아미오다론, 이뷰틸라이드와 비교한 무작위 연구에서도 대조군 대비 강력한 동율동 전환율을 보여주었다[26,28,101]. 아직 국내에서는 허가 전이나 향후 치료 효과가 기대된다.

이바브라딘(ivabradine)

동방결절의 funny 채널을 억제하여 심박수를 감소시키는 역할을 하며, 협심증이나 좌심실 구혈률 감소 심부전에서 예후 개선의 목적으로 사용이 허가된 약제이다[102]. 그러나 오히려 심방세동 발생을 15% 증가시키는 것으로 보고되어 약제 사용시 주의를 요한다[102].

비항부정맥제의 항부정맥 효과

레닌-안지오텐신 시스템 억제제

메타분석에 따르면 안지오텐신 전환효소 억제제나 안지오텐신 수용체 차단제는 좌심실 비대가 있는 고혈압 환자나 좌심실 구혈률 저하 심부전 환자, 또는 전기적 동율동 전환술을 시행받은 환자에서 심방세동의 새로운 발생을 감소시켰다[103,104]. 그러나 구조적 이상이 없는 발작성 심방세동 환자나 구조적 이상은 있으나 이차 예방 목적의 환자에서는 심방세동의 예방 또는 감소 효과를 일관되게 보이지 않았다[104,105]. 최근 심부전 치료제로 각광받고 있는 neprilysin 억제제는 추가적인 연구가 필요하겠으나 안지오텐신 전환효소 억제제 및 안지오텐신 수용체 차단제와 부정맥 감소 측면에서는 크게 다르지 않은 것으로 보이며[106], 알도스테론 길항제는 심방세동 예방에 효과가 있었다는 보고는 있으나[107,108], 현재로서는 심방세동 치료제로 권고할 정도의 근거는 충분하지 않다.

베타 차단제

베타 차단제는 심부전, 협심증, 고혈압 등 다양한 심혈관계 질환에서 흔히 쓰이는 약제로, 베타 수용체를 차단함으로써 전도계의 자동능을 낮추고 L형 칼슘 채널이나 funny 채널을 억제하여 항부정맥 효과를 나타낸다[94]. 동율동 상태의 좌심실 구혈률 감소 심부전 환자에서 심방세동의 새로운 발생을 감소시키는 것으로 메타분석에서 보고한 바 있으며[109], 증상이 있는 심방세동의 재발을 감소시킨 보고도 있으나[110,111] 심방세동의 심박수 조절보다 동율동 유지 효과인 것인지는 분명하지 않다.

항고지혈증제

스타틴 계열 약제는 특히 관상동맥 우회술 등의 심장 수술 후 심방세동 예방 효과로 관심을 끌었으나[112,113], 최근 대규모 무작위 연구에서 심장 수술 후 심근보호 효과나 심방세동 예방의 근거가 없으며 오히려 급성 신손상을 야기시킨다는 보고가 발표된 바 있어[114] 약제 사용에 신중을 요한다. 고도 불포화지방산은 지금까지의 연구에서 심방세동 예방의 충분한 근거는 부족하다[115-117].

결 론

현재까지 자료를 바탕으로 다음을 제시한다.
1. 항부정맥제의 선택은 동반 질환, 심혈관 위험도, 심각한 부정맥의 가능성, 심장외 독성, 환자의 선호도 그리고 증상의 정도 등을 고려하여 세심하게 판단해야 한다.
2. 항부정맥제를 투여받는 환자는 약제를 계속 투여받을 수 있는지 정기적인 평가가 필요하다.
3. 항부정맥제 처방 전 반드시 기저 심전도를 기록해놓는 것이 필요하다.
4. 항부정맥제 사용 중 심박수, QRS, QT 간격 연장과 방실 차단 발생에 대한 ECG 감시가 필요하다.
5. QT 간격 연장(500 ms), 의미 있는 동기능 장애 또는 방실결절 장애가 있고, 심박동기가 없는 환자에서 항부정맥제는 추천되지 않는다.
6. 항부정맥제 복용 중 약제 사용과 관련된 부정맥 발생 여부를 관찰하기 위하여 주기적으로 심전도 감시를 하는 것은 타당하다.
7. 플레카이니드, 프로파레논, 드로네다론 및 소타롤은 좌심실 기능이 정상이며, 병적인 좌심실 비대가 없는 환자에서 증상이 있는 재발성 심방세동의 예방을 위해 추천된다.
8. 소타롤은 심부전이 없는 안정적인 관상동맥 질환 환자에서 심방세동의 재발을 방지하기 위해 사용될 수 있다.
9. 소타롤은 QT 간격을 확인한 후 시작하여야 하고, 사용 초기에는 QT 간격의 연장에 주의를 기울여야 한다.
10. 드로네다론은 심부전이 없는 안정적인 관상동맥 질환 환자에서 심방세동의 재발을 방지하기 위해 권장된다.
11. 아미오다론은 심부전이 있는 환자에서 심방세동의 재발을 막기 위해 권장된다.
12. 아미오다론은 다른 항부정맥제보다 심방세동의 재발을 예방하는 데 더 효과적이지만 심장외 독성이 흔하고 이는 시간이 지날수록 증가한다. 따라서 다른 항부정맥제를 먼저 고려하여야 한다.
13. 베르나칼란트는 구조적 심질환이 없는 환자에서 새롭게 발생한 심방세동의 동율동 전환을 위하여 사용이 권고된다(최근 개발된 항부정맥제로 국내 허가 전이다).
14. 이바브라딘은 협심증이나 심부전에서 심박수를 감소하여 예후 개선을 시키는 약제이나, 오히려 심방세동 발생을 상승시키므로 주의가 필요하다.
15. 레닌-안지오텐신 시스템 억제제 및 베타차단제를 좌심실 구혈률이 감소된 심부전 환자에서 심방세동 발생의 예방을 위하여 처방하는 것은 타당하다.
16. 레닌-안지오텐신 시스템 억제제를 좌심실 비대가 동반된 고혈압 환자에서 심방세동 발생의 예방을 위하여 처방하는 것은 타당하다.
17. 항부정맥제를 복용하며 전기적 동율동 전환을 시행받는 심방세동 환자에서 재발을 예방하기 위하여 레닌-안지오텐신 시스템 억제제를 미리 처방하는 것을 고려해볼 수 있다.
18. 레닌-안지오텐신 시스템 억제제는 구조적 심질환이 없는 발작성 심방세동의 이차 예방을 위해 사용하는 것은 추천되지 않는다.
19. 알도스테론 길항제, 스타틴, 고도 불포화지방산은 근거가 부족하여 심방세동 예방 목적으로 처방은 추천되지 않는다.
