당뇨병성 신증의 최신 지견

Abstract

Diabetic nephropathy is the leading cause of end-stage renal disease, and is associated with increased risk of cardiovascular disease. Optimal control of blood glucose and blood pressure based on the blockade of renin-angiotensin system is the standard of care for the prevention and treatment of diabetic nephropathy. Regardless of these therapies, the prevalence of diabetic nephropathy continues to increase, highlighting the need for additional therapies. Diabetes affects the progression of kidney disease through a variety of mechanisms; several new therapeutic agents targeting these pathways have been developed, with currently being evaluated in clinical trials.

서 론

당뇨병성 신증은 말기신부전증의 가장 흔한 원인이며, 우리나라에서도 지속적으로 증가추세에 있다. 알부민뇨는 당뇨병성 신증의 질병 특이 표지자로서 신증의 진행을 잘 예측할 뿐만 아니라 심혈관계 합병증 발생의 독립적인 위험인자이기도 하다. 당뇨병성 신증의 치료 및 예방으로는 혈당 관리 및 레닌-안지오텐신-알도스테론계 억제제를 이용한 혈압 관리가 필수적이다. 그러나 이러한 치료에도 불구하고 일부에서는 지속적으로 진행하여 말기신부전증으로 진행하므로, 새로운 기전의 약제를 사용하여 신증의 진행을 억제하려는 시도들이 많이 시행되고 있다.

진 단

당뇨병성 신증은 알부민뇨와 신기능장애로 진단한다. 알부민뇨는 알부민 배설량에 따라 미세알부민뇨와 거대알부민뇨로 나뉜다. 미세 알부민뇨는 소변 알부민 배설이 20-200 ug/min (30-300 mg/day)이거나 무작위 소변의 알부민/크레아티닌비가 30-300 ug/mg이며, 거대알부민뇨는 그 이상으로 정의한다(Table 1). 운동, 감염, 발열, 심부전, 심한 고혈당 및 고혈압, 임신, 요로 감염, 혈뇨, 월경 등은 알부민뇨 배설을 증가시켜 당뇨병성 신증 진단을 부정확하게 할 수 있다. 따라서 3-6개월 내에 2회 검사를 더 시행하여, 총 3회의 검사 중 2회 이상의 이상 소견이 보여야 진단할 수 있다[1]. 신기능의 저하는 혈청 크레아티닌 수치를 이용하여 estimated glomerular filtration rate (eGFR)로 측정하는데, eGFR에 따라 신기능을 5단계로 구분한다(Table 2).

바이오마커

현재까지는 알부민뇨가 가장 잘 알려진 바이오마커이지만, 계속 증가하는 당뇨병성 신증을 고려할 때 알부민뇨 발생 이전부터 농도가 증가하여 신증의 예후를 예측할 수 있는 바이오마커를 규명하는 것이 신증 발생을 감소시킬 수 있는 중요한 방법이다. Neutrophil gelatinase associated lipocalin, N-acetyl-beta-glucosaminidase, cystatin C, alpha 1-microglubulin, Ig G, M, type IV collagen, nephrin, angiotensinogen, liver-type fatty acid-binding protein 등이 2형 당뇨병에서 신증의 조기 변화와 관련이 있다고 알려져 있다[2]. 그러나 이들은 대개 소규모 연구로서 조기 신증의 스크리닝 마커로서의 유용성을 확인하기 위한 대규모 전향적인 연구가 필요하다.

치 료

혈당조절

Diabetes Control and Complications Trials (DCCT) 연구에서는 1,441명의 제1형 당뇨병 환자들에게서 HbA1c를 7.3 vs. 9.1%로 조절했을 때, 6.5년간의 추적 관찰에서 적극적인 혈당 조절 군에서 미세 및 거대알부민뇨 발생률이 각각 39%, 54%로 유의하게 감소하였다[3]. 또한 DCCT 코호트를 18년간 연장한 Epidemiology of Diabetes Interventions and Complications 연구에서도 이러한 결과가 지속되었다[4]. 제2형 당뇨병 환자들을 대상으로 한 UK Prospective Diabetes Study (UKPDS) 연구에서는 적극적 치료군과 고식적 치료군의 10년간의 추적관찰 동안 발생한 HbA1c 11% 차이가 미세알부민뇨 발생 33% 감소와 연관이 있었다[5]. 이는 Action in Diabetes and Vascular disease 연구에서도 같아서, 11,140명의 제2형 당뇨병 환자들에서 적극적 치료군에서 미세 및 거대알부민뇨가 각각 9%, 30% 감소하였다[6]. 이러한 결과들에 근거하여 미국 당뇨병 학회에서는 당뇨병 환자의 혈당 조절을 HbA1c 7% 미만, 우리나라는 6.5% 미만으로 하도록 권고하고 있다.

혈압조절

혈압조절을 잘 할수록 당뇨병성 신증의 진행이 늦어진다는 것은 잘 알려져 있다. UKPDS에 따르면 혈압이 10 mmHg 감소할 때 당뇨병 합병증과 사망이 각각 12%, 15% 감소하는 것으로 알려졌다[7]. 레닌-안지오텐신계(renin angiotensin aldosterone system, RAS) 억제제는 다른 혈압약보다 신증 치료에서 우월한데, 이는 수출 사구체 세동맥(efferent arteriole)을 이완시켜 사구체내압을 줄이고 단백뇨를 감소시키기 때문이다. Micro-Albuminuria Reduction with Valsartan 연구에 따르면 다른 혈압약제와 비슷하게 혈압을 조절하여도 valsartan이 amlodipine에 비해 미세알부민뇨 감소효과가 더 뛰어났다(56 vs. 92%) [8]. 698명의 고혈압이 없는 미세단백뇨를 가진 1형 당뇨병 환자에서 angiotensin converting enzyme inhibitor (ACEI)는 거대알부민뇨로의 진행을 60% 억제하였으며, 정상으로 회복이 3배 더 많았다[9]. 이후 ACEI의 이러한 효과가 2형 당뇨병 환자에서도 확인되었다[10]. 안지오텐신 수용체 억제제(angiotensin II receptor blocker, ARB)를 사용한 여러 연구에서도 ACEI 연구 결과와 유사하여 알부민뇨의 진행, 신기능 악화 및 신부전증으로의 진행을 유의하게 억제하였다[11-13].

RAS 억제제들 간의 병합 치료

ACEI와 ARB 병합

ACEI나 ARB 사용에도 불구하고 신증의 일부는 계속 진행하는데, 이를 상쇄하기 위해 RAS 억제제 간의 병합요법이 시도되었다. 가장 대표적으로는 ACEI와 ARB 병합이다. Candesartan and Lisinopril Micro-albuminuria 연구에서는 candesartan과 lisinopril 병합 12주에 각각의 치료보다도 유의하게 미세알부민뇨 발생을 감소시켰다[14]. 그러나 이는 적은 수의 환자로 짧게 관찰한 연구였고, Ongoing Telmisartan Along and in Combination with Ramipril Global Endpoint Trial 연구에서는 5,620명의 심혈관계 합병증이 높고, 알부민뇨는 심하지 않은 환자를 대상으로(37.5% 당뇨) ramipril, telmisartan 단독 또는 병합으로 치료했을 때 투석, 크레아티닌 2배 이상 상승 및 사망의 복합 outcome이 병합군에서 유의하게 높았다(hazards ratio 1.09; 95% confidence interval 1.01-1.18, p < 0.037)[15]. 이는 병합군에서 혈압이 2-3 mmHg 더 감소하여 급성 신부전이 더 많았던 것에서 기인한다. 이후 Veterans Affairs Nephropathy in Diabetes 연구에서 1,448명의 2형 당뇨병성 신증에서 losartan과 lisinopril 병합군에서 renal outcome이 더 좋아지지 않고 고칼륨혈증, 급성신부전이 더 많이 생김이 다시 확인되어[16], ACEI와 ARB의 병합요법은 더 이상 권고되지 않고 있다.

ACEI/ARB와 레닌 억제제 또는 알도스테론 억제제의 병합

레닌 억제제가 RAS 억제제 사용으로 보상적으로 증가된 레닌 활성도를 감소시켜 도움이 될 것이란 가설하에 시행된 Aliskiren in the Evaluation of Proteinuria in Diabetes 연구는 losartan 최대 용량에 레닌 억제제인 aliskiren을 추가한 연구이다. 592명의 고혈압이 있는 신증환자에서 aliskiren은 혈압과 무관하게 소변알부민 배설량을 20% 감소시켰다[17]. 그러나 Aliskiren Trial in Type 2 Diabetes Using Cardio-Renal Endpoints 연구에서는 renal outcome 호전 없이 고칼륨혈증이 증가하여 조기 종료되었다[18]. 또한 알도스테론 억제제와 RAS 억제제의 병합 치료도 시행되었으며, 메타분석에서 단독 치료보다 단백뇨 감소 효과를 보였다[19]. 그러나 고칼슘혈증이 증가하며, 대규모 연구에서 신증의 진행이 호전되는 결과를 본 연구는 아직 없다. 이러한 결과들을 바탕으로 RAS 억제제들 간의 병합은 권고되지 않는다.

새로운 치료제들(Fig. 1)

당뇨병 치료제

최근 많이 사용되고 있는 dipeptidyl peptidase-4 억제제나 glucagon-like peptide 1 agonist는 신장의 나트륨 배설을 증가시킴으로써 신사구체 여과율을 감소시킨다. 6개월의 sitagliptin [20], 12주의 alogliptin 치료가 단백뇨를 감소시켰으나[21] 이러한 효과가 혈당 강하효과에 기인하는지, 신장에 미치는 직접적인 영향인지는 구별하기 어렵다. 4개의 3상 연구의 pooled analysis에서 RAS 억제제를 투여 받고 있는 217명의 당뇨병성 신증 환자에서 알부민뇨 감소가 HbA1c와 무관하게 관찰되어 신장 특이적인 효과를 지닐 가능성을 시사한다[22].
Sodium-glucose co-transporter 2 억제제는 신장 근위세뇨관에서 나트륨 재흡수를 억제하여 원위세뇨관의 나트륨 농도를 높임으로써 수입 사구체 소동맥(afferent arteriole)을 수축하게 만들어 GFR을 정상화시킨다. 또한 in vitro 연구에서 고혈당에 의한 염증 및 섬유화 반응도 억제할 수 있었다[23]. 임상 연구의 일차 변수는 아니지만 post hoc 분석에 의하면 canagliflozin [24], empagliflozin [25] 치료군에서 대조군 대비 GFR은 약간 감소하지만 계속 사용 시 호전되는 양상을 보이며, 알부민뇨는 지속적으로 감소하는 결과를 보였다. 이는 신증 초기에 사구체 여과율 증가를 교정함으로써 신장 보호 효과를 입증한 것이라 하겠다.

Bardoxolone methyl

당뇨병성 신증의 주된 기전은 산화적 스트레스와 염증반응으로서, 항산화, 항염증 제제인 bardoxolone methyl은 새로운 신증 치료제로서 많은 기대를 받았다. Bardoxolone methyl은 초기 연구에서는 GFR을 증가시켰으나[26], 후속 연구에서 GFR 15-30 mL/min/1.73 m2의 2,185명의 2형 당뇨병 환자를 대상으로 한 연구는 조기 종료되었다. GFR은 증가해도 만성 신부전이나 심혈관계 사망 감소 효과가 없고 심혈관계 질환 발생이 유의하게 증가되었기 때문이다[27]. Bardoxolone methyl이 왜 심혈관계 질환을 증가시키는지 확실하지 않으나 체액량 증가, 고혈압, 심부전 증가가 원인으로 생각된다.

Pirfenidone

만성신부전으로의 진행은 말기 변화인 네프론 파괴와 섬유화가 특징적이다. Pirfenidone은 항섬유화 제제로서 생쥐모델에서 좋은 효과를 보였다. 인체 대상 파일럿 연구에서 GFR을 증가시켰으나 통계학적인 유의성은 없었으며, 40%에 달하는 높은 중도 탈락률을 보여[28], 아직 신뢰성 있는 결과를 얻지 못하고 있다.

Vitamin D 수용체 활성화 제제

Vitamin D 수용체 활성화 제제는 항염증, 항단백 작용을 지닌다. Selective Vitamin D Receptor Activation with Paricalcitol for Reduction of Albuminuria in Patients with Type 2 Diabetes 연구에서는 2 ug/d의 paricalcitol이 고칼슘혈증의 부작용 없이 알부민뇨를 지속적으로 감소시키는 것이 확인되었다[29]. 이 연구를 토대로 Vitamin D 수용체 활성화 제제가 당뇨병성 신증의 진행을 억제하는지에 대한 후속 연구가 필요하다.

선택적 endothelin 수용체 억제제

신장 내 endothelin 수용체 활성화는 mesangial cell 증식, 염증, 섬유화를 촉진하며, 여러 연구에서 endothelin 수용체 A 억제는 실험적인 당뇨병성 신증을 호전시켰으며, avosentan 12주 사용한 임상 연구에서 알부민뇨가 감소되었다[30]. 이러한 결과는 후속 연구에서도 확인되었다[31]. 이는 1,392명의 RAS 억제제를 사용하던 당뇨병성 신증 환자에서 avosentan (25 or 50 mg) 또는 위약을 추가한 연구이다. 비록 avosentan이 단백뇨는 줄였지만 4개월 만에 조기 종료되었는데, 이는 체액 저류, 심부전과 같은 심혈관계 질환 발생이 유의하게 증가했기 때문이다. 이러한 부작용은 endothelin 수용체 B 억제에 의하므로, 현재 endothelin 수용체 A 선택성이 더 큰 atrasentan을 사용한 대규모 임상연구가 진행 중이다.

결 론

당뇨병성 신증은 알부민뇨라는 확실한 질병 마커가 존재하고 신증 기전의 주된 병인인 RAS 억제제를 사용함에도 불구하고 일부에서는 지속적으로 진행하여 만성 신부전증으로 진행하여, 당뇨병에 의한 만성 신부전증은 나날이 증가 일로에 있다. 이러한 관점에서 볼 때, 당뇨병성 신증 환자에서 혈당, 혈압, 고지혈증의 약물적 치료뿐만 아니라 흡연, 체중감소, 운동 등과 같은 생활습관 교정을 장기간 시행하여 신증을 위시한 미세혈관합병증을 60%, 사망이나 심혈관계 질환 발생을 50% 감소시킨 Steno-2 결과[32]를 재조명할 필요가 있다. 즉 현재까지는 당뇨병의 전반적 치료에서 요구되는 것과 같이 다양한 위험인자들을 모두 관리하는 것이 신증의 예후를 호전시키는 가장 좋은 방법이다. 그러나 한편으로는 신증의 병태생리에 근거한 신약 개발 및 더 나아가 신장 재생 치료, 즉 줄기세포와 같은 다른 측면의 치료제 개발에도 힘써야 할 것이다.
